Indications/Uses
Chronic hyperuricaemia in conditions where urate deposition has already occurred (including history or presence of tophus &/or gouty arthritis).
Dosage/Direction for Use
Initially 40 mg once daily, may be increased to 80 mg in patient who does not achieve serum uric acid <6 mg/dL after 2 wk.
Administration
May be taken with or without food: May be taken w/o regard to antacid use.
Special Precautions
Discontinue use if serious allergic/hypersensitivity reactions including SJS; gout flares occur. Avoid treatment in patients w/ preexisting major CV diseases eg, MI, stroke or unstable angina. Not to be started until an acute attack of gout has completely subsided. Flare prophylaxis for at least 6 mth w/ an NSAID or colchicine at initiation of treatment is recommended. Not recommended in patients in whom rate of urate formation is greatly increased eg, malignant disease & its treatment, Lesch-Nyhan syndrome; organ transplant recipients. Perform LFT prior to initiation of therapy & periodically thereafter. Patients w/ alteration of thyroid function. Not recommended in concomitant use w/ mercaptopurine/azathioprine. Not to be taken by patients w/ galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. Exercise caution before driving, using machinery or participating in dangerous activities. Not to be used during pregnancy & lactation.
Adverse Reactions
Gout flares; headache; diarrhea, nausea; liver function abnormalities; rash; edema.
Drug Interactions
May increase plasma conc of mercaptopurine/azathioprine. Metabolism may be increased & efficacy may be decreased w/ potent inducers of UGT enzymes.
